Creating tailored interventions to promote well-being, independence, and social inclusion. Offering counselling, crisis intervention, and practical assistance. Representing clients' interests and helping them access services like housing, healthcare, or legal aid Responding to concerns around child protection, elder abuse, or vulnerable adults. Coordinating with healthcare professionals, educators, legal services, and community organizations Linking clients to appropriate services and monitoring their progress.
